Insurance Coverage

Acupuncture might be covered by your health insurance plan.  Many plans do cover acupuncture, however this office makes no representation that yours does.  I am an out-of-network provider.  If your insurance plan does cover acupuncture, you are responsible for your annual deductible and coinsurance.  The  deductible is the amount per year that your insurance policy states must be paid by you before they will begin to pay any claims. The coinsurance is the percentage of the fees that the insurance company holds you responsible to pay after the deductible is met.  For each charge, your insurance company will reimburse what they consider reasonable and customary, which may be less than the standard billing fee.  If payment is received in a timely manner, I agree to accept the reasonable and customary fee as determined by your insurance company.
